1. A 52 year old male with epigastric pain that radiates to the back, nausea and vomiting. He claims to have felt unwell 3 days ago but thought it was a ‘stomach upset’ and has felt progressively worse. Blood tests reveal normal serum amylase, raised serum lipase and a calcium of 1.9 mmol/L.

 

 

2. The same man presents 6 weeks later with a fever, loss of appetite and an abdominal mass. Serum amylase is raised.
